All membership agreements require 7-days written notice to cancel or hold. This form will serve as your 7-days notice.
HOLD POLICY
When placing your membership on hold, any credit you have will be saved for use when your membership restarts. In order to do a hold, we must have an end date at which point your membership will automatically restart. There is a $15 per month hold charge. This keeps your information active in our system. CANCELLATION POLICY
If you are under 7 days, you agree to pay a $100 cancellation fee. All grandfathered membership rates will also be forfeited and returning members will be subject to current rates. Additionally, if you are on a discounted, contract membership, you will pay the difference between the monthly rate of the contract membership you selected and the membership you fulfilled.
